2. Behaviour Agreement - for both students and parent/guardians

Strathmore Blvd Church of Christ and its leaders, directors, officers, employees, contractors, agents, volunteers, members and representatives (collectively referred to as “Strathmore Blvd Church of Christ or SBCC”), are not responsible for any injury, loss or damage of any kind whatsoever sustained by any person or their property while participating in events, activities or travel with SBCC and all related activities associated with SBCC, including injury, loss or damage.

ASSUMPTION OF RISKS

IN CONSIDERATION OF Strathmore Blvd Church of Christ allowing me or my child to participate in events, activities, or travel with Strathmore Blvd Church of Christ and all related activities associated with Strathmore Blvd Church of Christ, including all events on and off the Strathmore Blvd Church of Christ property and all activities related to the Vacation Bible School (VBS, collectively referred to as the “Activities”), I acknowledge that I am aware of the possible Risks, Dangers and Hazards associated with participation in the Activities including the possible risk of severe or fatal injury to myself or others.

RELEASE OF LIABILITY and AGREEMENT

IN CONSIDERATION OF Strathmore Blvd Church of Christ allowing me or my child to participate in the Activities, I agree on behalf of myself and/or my child:

1. TO ASSUME and ACCEPT ALL RISKS arising out of, associated with or related to my or my child’s participation in the Activities. 

2. TO WAIVE and RELEASE Strathmore Blvd Church of Christ from any and all liability for any loss, damage, injury or expense that I or my child may suffer, or that my next of kin may suffer as a result of my or my child’s participation in the activities due to any cause whatsoever.

3. TO INDEMNIFY and HOLD HARMLESS Strathmore Blvd Church of Christ from any and all liability for any damage to the personal property of, or personal injury to, any third party resulting from my or my child’s participation in the activities.

4. TO INDEMNIFY and HOLD HARMLESS Strathmore Blvd Church of Christ from any and all claims, demands, actions and costs for any loss, injury, damage or expense whatsoever that might arise out of my or my child’s participation in the Activities. 

CHILD PARTICPATION CONSENT

Acknowledgment of Participant:

I, the undersigned Participant, understand that I am responsible to act in a safe and responsible fashion, to follow the instructions or directions of the persons in charge of the VBS, and to obey requests to comply with safety regulations as directed by the persons in charge of the VBS, including designated leaders and drivers of private or public transportation. I will be solely responsible for myself, will wear a seat-belt when available and will not disturb or distract the driver when using private or public transportation to travel to and from VBS activities. At all VBS sports events or other activities, I acknowledge that it is my responsibility to obtain and wear appropriate safety equipment. I will not endanger the safety of others or myself at any activities, outings or sports events of the VBS or when using private or public transportation for travel to and from such activities. I also understand that I may be photographed or appear in video for such purposes as Strathmore Blvd Church of Christ deems necessary.

Acknowledgment of Parent or Guardian of Participant:

We, the undersigned Parents or Guardians of the Participant, hereby authorize and consent to the Participant’s involvement in the VBS, including any use of private or public transportation deemed necessary by the persons in charge of the VBS for Participant travel to and from VBS activities, or to the NEAREST SUITABLE MEDICAL or HOSPITAL FACILITY in the event that emergency or other medical treatment not available at the site of a VBS activity is deemed advisable. We hereby consent to and authorize such emergency or other medical treatment of the Participant as may be deemed advisable in the event of accident, injury, or illness during the activities of the VBS. We also understand that the participant may be photographed or appear in video for such purposes as the SBCC deems necessary.


2. GUIDELINES AND BEHAVIOUR AGREEMENT

VBS RULES/GUIDELINES

1. Age Eligibility

- Ages accepted are from 3 yrs to 12 yrs old. Registration is subject to available space

- Children must be potty-trained if under 5 years unless accompanied by a parent/guardian or care-giver who will stay for the whole duration of the VBS class of each day.

2. Registration Requirements:

- Pre-registration is required. Registration is subject to available space.

- Parents/guardians MUST fill out the VBS Waiver, Medical Release form and Behaviour Agreement.

3. Check-in/Check-out System:

- Secure check-in with name tags for kids and matching pickup tags for parents/guardians.

- Only authorized individuals listed on the registration form can pick up the child.

4. Drop-off Time:

- Drop-off time is between 9:00 AM to 9:15 AM.

- Late arrivals may not be allowed after a certain point for safety reasons.

5. Pick-up Time:

- Children must be picked up promptly at 12:00 PM after VBS ends.

6. Behaviour expectations:

- Parents/guardians need to sign off on a Behaviour Agreement.

- Repeated misbehaviour can result in dismissal from the program.

7. Health & Safety:

- Sick children (fever, cough, contagious illness) MUST stay home.

- Medical Release form must be filled out upon registration.

8. Safety & Supervision

- All volunteer VBS teachers and Teacher Helpers MUST have current Vulnerable Sector Check certificate from the Toronto Police Service.

- Two-deep leadership rule must be followed (Children of each age group MUST be supervised by two adults at all times)

Ages 3–4 2 adults per 5–6 children

Ages 5–8 2 adults per 6–8 children

Ages 9–10 2 adults per 8–10 children

Ages 11-12 2 adults per 8-10 children

- Presence of Floaters or Support – they are extra adults to help with transitions, bathrooms, and emergencies.


Behaviour Agreement for Students

BEHAVIOUR AGREEMENT

Purpose: 

To ensure that all children have a safe, fun, and respectful environment during Vacation Bible School.

Expectations for All Participants:

1. Respect Others

o Speak kindly and use appropriate language.

o Listen when others are speaking.

o Keep hands and feet to yourself.

2. Respect Teachers/Leaders and Volunteers

o Follow instructions the first time given.

o Be polite and cooperative.

3. Respect Property

o Use supplies and materials appropriately.

o Keep the church and classrooms clean and tidy.

4. Be Safe

o Stay with your group.

o Walk (don’t run) inside buildings.

o Report any unsafe behavior to a leader.

5. Participate Fully

o Be on time and ready to join activities.

o Try your best and encourage others.

Behaviour Consequences:

If a child does not follow the behaviour expectations, the following steps will be taken:

1. Verbal Reminder

o The teacher/leader will remind the child of the expected behaviour.

2. Time Out / Cool Down Period

o The child may be asked to sit out of an activity for a short time.

3. Parent/Guardian Notification

o If behaviour continues, a leader will speak with the parent/guardian.

4. Removal from Program (if necessary)

o In cases of repeated or serious misconduct, the child may be removed from VBS.
